Monteggia fracture. Reduction of the ulna under anesthesia, which should reduce the radial head. If not, open reduction of both.

I agree that it would be desirable to see a lateral elbow, but the diagnosis of Monteggia can be made with the available images; the radial head is clearly dislocated in the first photo

This is GS # ulna . Needs Osteoclasis under GA and Cast immobilization. There is no radial head dislocation as can be judged by its relation to capitullum which is constant

The radial axis doesn't intersect the center of the capitellar nucleus; it's a Monteggia

Type2 monteggia #. Close reduction and pop cast.

Tapas Talukdar
Paediatric Monteggia/ Montrggia equovalent. Close reduction and k wire. k wire should pierce the cortex of distal fragment. No need to reconstruct annular lig. Radial head will reduce spontaniously.
